Heartsore
A story-led approach to the transition into adult healthcare.

Overview
Heartsore is a narrative-based intervention for young people making the transition from paediatric to adult healthcare.
Rather than approaching transition as another set of instructions, the project uses fictional characters and story to explore the less visible experience of taking increasing responsibility for a lifelong health condition: identity, independence, relationships, uncertainty and self-management.
Narrative approach
The project treats narrative as an experiential space: a way to encounter recognisable characters, difficult choices and the consequences that follow.
